Microbubble Hydrotherapy Client Acknowledgment & Release

By booking and participating in a THRIVEnaturally Pet Wellness microbubble hydrotherapy session, I acknowledge and agree to the following:

Pet Health & Suitability
I understand that microbubble hydrotherapy is a bathing and skin-wellness service and is not veterinary medical care, diagnosis, or treatment. I am responsible for providing accurate information about my pet’s health, skin condition, allergies, medications, sensitivities, behavioral concerns, and any other information that may affect the safety or suitability of the session.

I understand that individual results vary and that no specific health or skin outcome is guaranteed.

Pet Supervision & Participation
I understand that I am required to remain present and available throughout my pet’s session. I will follow reasonable instructions provided by THRIVEnaturally Pet Wellness regarding my pet’s handling, comfort, and safety.

I understand that the session may be stopped or modified if my pet becomes excessively stressed, distressed, aggressive, or otherwise appears unable to safely continue.

Risks & Acknowledgment
I understand that, as with any bathing or handling service, there may be risks including, but not limited to, stress, discomfort, skin irritation, allergic or sensitivity reactions, slipping or injury, or aggravation of an existing skin condition.

I voluntarily choose to have my pet participate and acknowledge these inherent risks.

Release of Liability
To the fullest extent permitted by applicable law, I agree to release and hold harmless THRIVEnaturally Pet Wellness and its owner/service providers from claims arising from my pet’s participation in the session, except to the extent caused by conduct for which liability cannot legally be waived or released.

I understand that I remain responsible for my pet’s actions and for any information I provide regarding my pet.

Emergency Care
If an unexpected health or safety concern arises during the session, I authorize THRIVEnaturally Pet Wellness to take reasonable steps to protect my pet, including contacting me and, when reasonably necessary, seeking veterinary assistance. I understand that any veterinary or emergency care costs are my responsibility.
